Seafarer Medical Requirements in United States
In the United States, the U.S. Coast Guard National Maritime Center (NMC) evaluates mariners' medical fitness and issues the merchant mariner medical certificate.

Applicable standards
Medical standards are set in federal regulations (46 CFR) and explained in the Coast Guard's Merchant Mariner Medical Manual and NMC guidance on medical fitness for certification.
Examination information
Applicants submit the application for a medical certificate (Form CG-719B). A licensed medical practitioner records the physical examination on Form CG-719K (or CG-719KE for entry-level ratings, where applicable). The NMC evaluates the application.
Approved / authorized medical examiners
The examination is performed by a licensed medical practitioner (for example a physician, physician assistant or nurse practitioner) as described in NMC guidance; the Coast Guard, not the examining practitioner, issues the certificate.
Medical certificate
The medical certificate is separate from the Merchant Mariner Credential. Its validity depends on the credential and endorsements held; check NMC guidance for your situation.

Renewal
